ADOPTION FACILITATION SERVICES WILL PROVIDE THE FOLLOWING THROUGH DIRECT, REFERRALS AND RESOURCES:                    

*Explain options to Birthmother choosing independent adoption
*Educate Adoptive Parent(s) regarding independent adoption
  and the home study process. 
*Actively work with Birthmother and Adoptive Parent(s) to
  secure a compatible match.
*Adoption outreach and awareness to prospective Adoptive
  Parent(s) and Birthmothers through necessary means
  including, but not limited to, internet, telephone publication,
  periodicals, community outreach and healthcare facilities.
*Educate Adoptive Parent(s) and Birthmothers on the lifelong
  impact of adoption.
*Provide prescreening of Adoptive Parent(s) and Birthmothers
  regarding their adoption commitment.
*Obtain proof of pregnancy from Birthmother's medical facility.
*Provide and obtain a completed applications from both
  Adoptive Parent(s) and Birthmother that includes medical
  history, education, social and familial history.
*Develop individual  reporie between AFS, Adoptive Parent(s)
  and Birthmother.
*Assist Adoptive Parent(s) with Dear Birthmother letter.
*Assist both parties with their adoption plan..
*Arrange for professional, adoption counseling for both parties,
  if necessary.
*Obtain ethical legal counsel to generate all necessary
  paperwork, advisements and other legal services, as necessary.
*Provide a working relationship with all necessary adoption 
  professionals to ensure a smooth transition, per situation. 
*Arrange maternity home or other living solutions for
  Birthmother, if necessary.
*Arrange for reasonable Birthmother living expenses, if
  necessary.
*Referral to Medi-Cal or MediCaid assistance, if necessary.
*Arrange transportation for all parties involved, if necessary.
*Assist with hospital arrangements for Birthmother.
*Attendance at hospital during labor and delivery, if requested
  by Birthmother.
*Support for Adoptive Parent(s) at hospital during labor and
  delivery, if requested.
*Follow-up during hospital stay with Birthmother.
*Transportation home for Birthmother, if requested.
*Provide printed materials including medical, invitro drug
  effects, cigarette  smoking hazards, fetal alcohol effects, etc.
*Referrals to adoptive parent support groups.
*Referrals to adoption educational classes, seminars and
  conferences.
*Indefinite open adoption liaison for Birthmother and Adoptive
  Parent(s) for photo, letter exchanges and visitation.
*Ongoing post adopt services for all parties
*Referencecs provided.
